Keto diet in Pakistan: a desi keto plan, and is it safe?

Keto cuts carbohydrate to about 20 to 50 g a day: no roti, rice, aloo, fruit or sugar, and meals built on eggs, chicken, fish, paneer and non-starchy sabzi. It can cause fast early weight loss, but after a year results are similar to other diets. It is not safe for everyone, especially people on diabetes medicines.

High-protein desi gym meal: chicken, boiled eggs, rice, daal and a glass of milk

What a desi keto plate looks like

  • Protein: eggs, chicken, fish, mutton in moderation, paneer.
  • Low-carb sabzi: palak, gobhi, band gobhi, bhindi, lauki, tori, shimla mirch, kheera, salad.
  • Fats: cooking oil or ghee in measured amounts, nuts, a little cheese.
  • Small amounts: plain dahi, tamatar, pyaz.
  • Avoid: roti, rice, naan, daal and chana in large portions, aloo, fruit, sugar, honey, juices, bakery items.

Common side effects

In the first week many people get "keto flu": headache, tiredness, dizziness and irritability, often from losing water and salt. Constipation, bad breath and muscle cramps are common. Over months, some people see LDL cholesterol rise sharply; check a lipid profile after 2 to 3 months.

A more sustainable option for most Pakistanis

Most families eat roti and rice every day. A lower-carb but balanced plan, with one chapati per meal, half the plate sabzi and protein at every meal, gives steady weight loss without cutting whole food groups. See the Pakistani diet plan for weight loss.

Do not start keto without your doctor if

  • You have type 1 diabetes, or take insulin, sulfonylureas or SGLT2 tablets (empagliflozin, dapagliflozin)
  • You are pregnant or breastfeeding
  • You have kidney disease, gallbladder or pancreas problems, or very high cholesterol
  • You have had an eating disorder
  • You are under 18

In these cases a general chart can be wrong for you. Book an online consultation.

Keto diet diet: common questions

What is the keto diet?

A ketogenic diet cuts carbohydrate to roughly 20 to 50 g a day, so the body burns fat and makes ketones for fuel. In Pakistani terms, that means almost no roti, rice, daal, aloo, fruit or sugar, and meals built on eggs, meat, fish, paneer, non-starchy sabzi and some fat.

Is keto good for weight loss?

It can give fast early weight loss, partly water. In studies lasting a year, keto and other diets that cut the same calories give similar weight loss. The best diet is the one you can follow; many people in Pakistan find keto hard with family meals built on roti and rice.

Is keto safe?

For healthy adults, a few months under guidance is usually safe. It can raise LDL cholesterol in some people, cause constipation, tiredness ("keto flu"), and kidney stones, and it is risky with some medicines. It is not suitable for everyone: see the "do not start" list on this page.

How do I get fibre on keto?

From non-starchy sabzi (palak, gobhi, bhindi, lauki, shimla mirch), salads, a handful of nuts, chia seeds and isabgol husk. Drink plenty of water. Constipation is common on keto without these.

Can I do keto with diabetes?

Only with your doctor. Low-carb eating can lower sugar quickly, so insulin or sulfonylurea doses may need to come down to avoid lows, and people on SGLT2 tablets (gliflozins) can develop ketoacidosis. Type 1 diabetes needs specialist care.

Do I need a keto dietitian?

If you have a medical condition, take medicines, or want to stay on keto for months, yes. A dietitian checks your reports, keeps protein and fibre adequate, and plans a safe way back to normal eating.
