TSH normal range, and what high or low TSH means

For most adults the normal TSH range is about 0.4 to 4.0 mIU/L. A high TSH usually means an underactive thyroid, and a low TSH an overactive one or too high a tablet dose. Diet does not treat thyroid disease, but iodised salt and correct tablet timing matter.

TSH results and what they usually mean

TSHWith free T4Usually means
0.4 to 4.0 mIU/LNormalNormal thyroid function
Above rangeLow T4Hypothyroidism (underactive)
Mildly above rangeNormal T4Subclinical hypothyroidism: doctor decides on treatment
Below rangeHigh T4Hyperthyroidism (overactive)
Below rangeNormal T4Subclinical hyperthyroidism, or too high a tablet dose

TSH alone is not enough; doctors read it with free T4 and sometimes antibodies.

Taking levothyroxine correctly

  • On an empty stomach with plain water, usually in the morning.
  • Wait 30 to 60 minutes before chai, milk or breakfast.
  • Keep calcium and iron tablets about 4 hours away.
  • Take it at the same time every day, including weekends.

Wrong timing is one of the most common reasons TSH stays high despite tablets.

Diet points that do matter

  • Iodised salt: use it in normal amounts; iodine deficiency is still present in parts of Pakistan.
  • No iodine or kelp supplements unless prescribed; too much iodine can worsen thyroid problems.
  • Cabbage, gobhi and sarson are fine cooked and in normal amounts when iodine intake is adequate.
  • Selenium, zinc and iron support thyroid function; get them from eggs, meat, daal and nuts rather than pills.

The full plan is on the thyroid diet chart page.

See a doctor first if

  • Your TSH is outside your lab's range
  • You are pregnant or planning pregnancy (targets are different)
  • You have palpitations, tremor, or sudden weight change
  • You have a lump or swelling in the neck, or trouble swallowing

Common questions

What is the normal TSH range?

Most laboratories use about 0.4 to 4.0 mIU/L for adults, and some report up to 4.5. Pregnancy has its own narrower ranges by trimester. Always read the range printed on your own report, because methods differ between labs.

What does a high TSH mean?

A high TSH usually means an underactive thyroid (hypothyroidism): the gland is not making enough hormone, so the brain sends more TSH to push it. A mildly high TSH with normal T4 is called subclinical hypothyroidism, and your doctor decides whether it needs treatment.

What does a low TSH mean?

Usually an overactive thyroid (hyperthyroidism), or too high a dose of thyroid tablets. Symptoms can include weight loss, palpitations, tremor, heat intolerance and anxiety.

Does diet change TSH?

Food does not treat thyroid disease; the tablet does. But diet matters in three ways: use iodised salt (iodine deficiency raises TSH), do not take extra iodine or kelp supplements, and take your levothyroxine on an empty stomach, away from milk, chai, calcium and iron.

When should TSH be rechecked?

Usually 6 to 8 weeks after starting or changing a dose, then every 6 to 12 months once stable, as your doctor advises. Testing earlier than 6 weeks gives a misleading result.

Can I lose weight with a high TSH?

Weight loss is slower while thyroid levels are off, and it becomes normal once TSH is in range on the right dose. Untreated hypothyroidism usually adds only a few kilos, mostly fluid, so the thyroid is rarely the whole reason for large weight gain.
