Fatty liver diet chart: symptoms, grades and what to eat
Fatty liver usually has no symptoms and is found on an ultrasound or a high SGPT test. The main treatment is food and activity: lose 5 to 10 percent of your weight slowly, cut sugary drinks, sweets and fried food, eat more sabzi, daal, whole grains and fish, and walk most days.
Why fat builds up in the liver
The liver stores extra energy as fat when we regularly eat more than we use, especially sugar and refined carbohydrate. Belly fat and insulin resistance push more fat into the liver. That is why fatty liver is common in people with prediabetes, diabetes, PCOS, high triglycerides and high blood pressure, and why it improves with the same steps that improve those conditions. Doctors now often call it MASLD (metabolic dysfunction-associated steatotic liver disease).
6 rules of a fatty liver diet
- Lose weight slowly: about ½ to 1 kg a week until you have lost 5 to 10 percent. Crash diets can worsen the liver.
- Stop sugary drinks first: cold drinks, packaged juice, sweet lassi and sweet chai.
- Choose whole grains: chakki atta, daliya, oats and brown rice in measured portions.
- Healthy fats in small amounts: less ghee and fried food; nuts, fish and a little olive or canola oil.
- Protein at every meal: daal, chana, eggs, chicken, fish and dahi protect muscle while you lose fat.
- Move: 150 minutes a week of brisk walking, plus strength exercise twice a week, lowers liver fat even before the scale moves.

Tests that show progress
Your doctor may repeat SGPT (ALT), sugar, HbA1c and lipids every few months, and the ultrasound later. Falling SGPT and weight usually show the diet is working. See what the numbers mean on the SGPT normal range page.
Check your BMI
Uses the Asian BMI ranges, which are lower than the Western ones because health risks start at a lower weight in South Asians.
BMI is a screening number, not a diagnosis. It does not separate muscle from fat, and it is not used in pregnancy.
Send my BMI to a dietitianSee a doctor first, or go now, if
- You have yellow eyes or skin, a swollen belly, confusion, or vomiting blood (go to hospital)
- You have hepatitis B or C, or a liver doctor has found cirrhosis
- You have diabetes and take insulin or sugar tablets
- You drink alcohol regularly
In these cases a general chart can be wrong for you. Book an online consultation.
Fatty liver diet: common questions
What are the symptoms of fatty liver?
Most people have none, and fatty liver is found on an ultrasound or a raised SGPT (ALT) test. Some feel tired or have a dull discomfort under the right ribs. Yellow eyes, swelling of the belly or legs, or vomiting blood are signs of advanced liver disease and need urgent care.
Can fatty liver be reversed with diet?
Often, yes, in the early stages. Losing about 5 percent of body weight reduces liver fat, and 7 to 10 percent can also reduce inflammation and early scarring. Diet, activity and controlling sugar and cholesterol are the main treatment.
What does grade 1, 2 or 3 fatty liver mean?
It is the ultrasound grade of how much fat the liver shows: grade 1 mild, grade 2 moderate, grade 3 severe. The grade does not show scarring; your doctor may order other tests (such as FibroScan or blood scores) to check that.
Is coffee good for fatty liver?
Studies link regular coffee (without much sugar) to less liver scarring. Two cups a day is reasonable for most adults unless coffee affects your sleep, heart or acidity.
Which foods are worst for fatty liver?
Sugary drinks and juices, sweets and bakery items, refined flour in large amounts, fried food and alcohol. Fructose from sugary drinks is especially linked to liver fat.
Can a thin person have fatty liver?
Yes. South Asians can have fatty liver at a normal BMI, often with belly fat, insulin resistance or prediabetes. The same diet rules apply, with more focus on sugar and activity than on weight.
