SGPT (ALT) normal range, and what a high SGPT means
SGPT, also called ALT, is a liver enzyme. Most labs give a normal range up to about 40 U/L; experts suggest a healthy upper limit closer to 33 U/L for men and 25 U/L for women. A high SGPT means liver cells are irritated, most often from fatty liver or hepatitis.
SGPT levels and what they usually mean
| SGPT (ALT) | Usually means |
|---|---|
| Within your lab's range | Normal |
| Up to about 2 to 3 times the upper limit | Mild rise: common with fatty liver, some medicines, or after heavy exercise |
| About 3 to 10 times | Needs tests soon: hepatitis, drug effect, other liver disease |
| More than 10 times | Acute liver injury (for example acute hepatitis): see a doctor promptly |
A guide only. Your doctor reads SGPT together with SGOT (AST), bilirubin, alkaline phosphatase, hepatitis tests and ultrasound.
Food steps that help a high SGPT
- Lose weight slowly if overweight: 5 to 10 percent over a few months.
- Stop cold drinks, packaged juices, sweets and bakery items.
- Choose chakki atta, daliya and oats over maida; more sabzi, daal, fish and fruit.
- Avoid alcohol and do not take herbal or "liver tonic" products unless your doctor approves.
- Coffee without much sugar, two cups a day, is linked to a healthier liver.

See a doctor first if
- SGPT is more than 3 times the upper limit, or keeps rising
- You have yellow eyes, dark urine, pale stools or itching
- You have not been tested for hepatitis B and C
- You started a new medicine or herbal remedy recently
Common questions
What is the normal range of SGPT (ALT)?
Most Pakistani labs print an upper limit of about 40 to 45 U/L. The American College of Gastroenterology suggests a lower "healthy" upper limit of about 29 to 33 U/L for men and 19 to 25 U/L for women. Always compare with the range printed on your own report.
Are SGPT and ALT the same?
Yes. SGPT (serum glutamic pyruvic transaminase) is the older name; ALT (alanine aminotransferase) is the newer one. SGOT is the old name for AST.
What causes high SGPT?
In Pakistan the most common causes are fatty liver and hepatitis B or C. Others include alcohol, some medicines (TB drugs, high-dose paracetamol, some herbal and "desi" remedies), heavy exercise or muscle injury, thyroid and celiac disease. Your doctor decides which tests are needed.
How can I lower SGPT with diet?
If the cause is fatty liver, losing 5 to 10 percent of your weight, cutting sugary drinks, sweets and fried food, and walking most days usually lowers SGPT over 3 to 6 months. Avoid unprescribed herbal medicines and alcohol.
Is SGPT 60 or 80 dangerous?
Mild rises (up to about 2 to 3 times the upper limit) are common with fatty liver and are not an emergency, but they need a cause found. Very high levels (several hundred or more), or any rise with yellow eyes, need prompt medical care.
