Insulin resistance diet: symptoms, test and a Pakistani diet chart

Insulin resistance means your body needs more insulin than normal to keep blood sugar in range. It often comes with belly fat, dark skin patches on the neck, PCOS, fatty liver or prediabetes. It usually improves with losing 5 to 7 percent of body weight, measured roti and rice, more protein and sabzi, no sugary drinks, and walking most days.

What is insulin resistance?

After a meal, sugar from food enters the blood, and insulin from the pancreas moves it into muscle, liver and fat cells. In insulin resistance these cells respond poorly, so the pancreas makes more and more insulin to keep sugar normal. For years the blood sugar can look fine while insulin stays high. Belly fat, fat in the liver, inactivity, poor sleep, family history and South Asian genes all push it up. Over time the pancreas cannot keep up, and prediabetes and then type 2 diabetes follow.

Signs and tests

Sign or testPoints to insulin resistance when
Waist (South Asian cut-off)90 cm (35.5 in) or more in men, 80 cm (31.5 in) or more in women
SkinDark, velvety patches on the neck, armpits or knuckles; many skin tags
Fasting sugar / HbA1c100 to 125 mg/dL, or 5.7 to 6.4% (prediabetes)
Triglycerides and HDLTriglycerides 150 mg/dL or more; HDL below 40 (men) or 50 (women)
Other conditionsPCOS, fatty liver, high blood pressure, diabetes in pregnancy

6 rules of an insulin resistance diet

  1. Aim for 5 to 7 percent weight loss if you are overweight, at ½ to 1 kg a week. For an 80 kg person, that is 4 to 6 kg.
  2. Measure the carbohydrate: 1 chapati or ½ plate rice per meal, not both, and not seconds.
  3. Protein at every meal: eggs, daal, chana, chicken, fish or dahi. It keeps you full and blunts the sugar rise.
  4. Half the plate sabzi and salad, and eat it first. Fibre slows sugar absorption.
  5. Drink no sugar: juice, cold drinks, sharbat and sweet chai raise insulin fastest.
  6. Move every day: 150 minutes of brisk walking a week plus 2 sessions of strength exercise (squats, wall push-ups, resistance bands). A 10-minute walk after meals helps sugar right away.

Pakistani food swaps for insulin resistance

Small changes to everyday food, without a separate menu.

Instead ofTry
White rice in a full plate½ plate rice with daal and salad, or 1 chapati
Maida bread, rusk, biscuits, bakeryChakki-atta chapati, oats, roasted chanay
Juice, cold drinks, sweet lassi, sharbatWater, lemon water, plain lassi, whole fruit
Sweet chai 4 to 5 times a day2 cups, little or no sugar
Aloo bhujia or aloo parathaMixed sabzi with chana, or an egg paratha made thin with little oil
Mithai after mealsA fruit, or mithai only on occasions in a small piece

Sleep, stress and timing

Short sleep (under 6 hours) and late, heavy dinners make insulin resistance worse. Aim for 7 hours, finish dinner 2 to 3 hours before bed, and keep meals at regular times. Women with irregular periods should also read the PCOS diet plan, and anyone with a fatty liver on ultrasound the fatty liver diet.

Check your BMI

Uses the Asian BMI ranges, which are lower than the Western ones because health risks start at a lower weight in South Asians.

Talk to a dietitian first if

  • You already have diabetes and take insulin or sugar tablets (your meals must match your medicines)
  • You are pregnant or breastfeeding
  • You have kidney disease (protein and potassium need a personal plan)
  • You have an eating disorder, or are under 18

In these cases a general chart can be wrong for you. Book an online consultation.

Insulin resistance diet: common questions

What are the symptoms of insulin resistance?

Insulin resistance itself often has no symptoms. Signs that point to it are belly fat (waist 90 cm or more in men, 80 cm or more in women for South Asians), dark velvety skin on the neck, armpits or knuckles (acanthosis nigricans), skin tags, irregular periods or PCOS, fatty liver, high triglycerides, low HDL, high blood pressure, and sugar in the prediabetes range. Some people feel sleepy or hungry soon after carbohydrate-heavy meals, but that alone does not prove it.

Is there a test for insulin resistance?

There is no single routine test with an agreed cut-off. Doctors usually judge it from your waist, fasting sugar, HbA1c, triglycerides and HDL, and conditions like PCOS or fatty liver. Some labs offer fasting insulin and a HOMA-IR score; it can support the picture, but labs and studies use different cut-offs, so it should not be read on its own.

Can insulin resistance be reversed?

It can improve a lot. Losing 5 to 7 percent of body weight and doing about 150 minutes of brisk walking a week lowered the risk of type 2 diabetes by 58 percent in the US Diabetes Prevention Program. Muscle-building exercise helps too, because muscle uses sugar. Improvement starts within weeks, before the scale moves much.

Is insulin resistance the same as prediabetes?

No, but they are linked. Insulin resistance usually comes first: the pancreas makes extra insulin to keep sugar normal. When it can no longer keep up, sugar rises into the prediabetes range (fasting 100 to 125 mg/dL or HbA1c 5.7 to 6.4%) and later into diabetes.

Do I need a keto or no-roti diet?

No. Lower-carbohydrate eating can help, but very strict diets are hard to keep and are not needed. Measured portions of whole grains (1 chapati or ½ plate rice per meal), more protein, sabzi and daal, and no sugary drinks work for most people and fit Pakistani home food.

Does metformin treat insulin resistance?

Doctors sometimes prescribe metformin for prediabetes or PCOS with insulin resistance. It is a medicine decision for your doctor, and it works alongside, not instead of, diet and activity. Long-term metformin can lower vitamin B12, so ask about a check.
